Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on July 16, 2019 from 12:00 p.m. until 4:15 p.m. At the time of entrance, the following adult to child ratios were observed: 6 children ages 2 to 3 years, with 1 staff (napping) 6 children age 3 years, with 1 staff (napping) 9 children age3 years, with 1 staff (napping) 8 children age 4 years, with 1 staff (napping) 7 children age 4 years, with 1 staff (napping) 6 children age 4 years, with 1 staff 9 children ages 5 to 6 years, with 1 staff 8 children ages 5 to 6 years, with 1 staff 14 children ages 5 to 6 years, with 2 staff 11 children age 6 years, with 1 staff The records sample size consisted of 5 children?s records and 37 staff records. Children were observed resting quietly at nap time, eating lunch which children bring from home each day, and playing with games and in learning centers. Medication administration was not observed today. However, a sampling of 5 medications were reviewed with regard to storage, labeling, written authorizations and required documentation. One accident / injury reports was reviewed for a recent serious injury that was reported to the inspector. The facility to include all areas used by children on care, inside and out, were inspected. Areas of non-compliance are identified on the violation notice. Transportation was not observed during the inspection. However, standards regarding transportation were reviewed. Standard #: 63.2(17)-1720.1-A
Description: Based on a review of records and interview, the vendor did not ensure that every staff supervising children has documentation of the results of Search of the Central Registry within 30 days of employment or beginning to serve as a volunteer and every five years thereafter. Evidence: 1. Thirty Seven currently employed staff members' records were reviewed. 2. The records for 10 of the 37 staff records reviewed contained signed sworn disclosure forms that were dated more than 5 years ago: (staff #5 - 8/6/12; staff #10 - 9/9/12; staff #13 - 7/5/12; staff #16 - 7/3/12; staff #21 - 8/9/12; staff #24 - 8/3/12; staff #26 - 8/8/12; staff #28 - 8/6/12; staff #31 - 8/9/13; staff #35 - 8/6/12). 3. The records for 9 of the 37 staff records reviewed contained signed results of the central registry search were dated more than 5 years ago: (staff #5 - 10/17/13; staff #10 -10/31/12; staff #13 - 9/5/12; staff #21 - 8/9/12; staff #24 - 9/5/12; staff #26 - 9/5/12; staff #28 - 9/5/12; staff #31 - 10/17/13; staff #35 - 9/5/12). 4. The program director (staff #38) confirmed that this documentation was not available during the inspection.

Plan of Correction: These background checks are being updated and the results of the Central registry Searches will be forwarded to the licensing inspector upon receipt. Updated Sworn Disclosure forms have been signed , but they could not be located during the inspection. These will be forwarded to the inspector and filed in each staff's record for future reviews.
