Comments:
An unannounced monitoring inspection was conducted on September 13, 2023. This on-site visit was conducted from 11:30 a.m. through 12:15 p.m. Upon arrival, there were 25 children, ages 4 through 5, in the care of 3 staff members. Four children and four staff records were reviewed.

Please complete your plan of correction within 5 business days and return to the licensing inspector.

Based on record review the facility failed to obtain updated background checks for two employees.
Evidence: 1. Record review revealed Staff #1 had not obtained an updated sworn statement, fingerprint check and a Virginia central registry search. 2. Staff #1's sworn statement was documented on 9/27/2017. Staff #1's fingerprint check was documented on 06/07/2018. Staff #1's Virginia central registry search was documented on 6/7/2023. 3. Staff #2's fingerprint check was documented on 6/06/2018. Staff #2's Virginia central registry check was documented on 11/1/2017. 4. Administration confirmed updated background checks had not been initiated.
Plan of Correction:
Both staff members have gone and completed the fingerprint check, Virginia central registry search, and sworn statement. Once we have obtained the items listed above we will be sure to send them over.
