Comments:
An annual monitoring inspection was initiated on 04/20/2023 and concluded on 04/20/2023. The inspector was on site at the center from approximately 10:20am to 12:20pm. There were 5 children in care with 1 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and classroom was inspected today. Five children?s records and five employee records were reviewed during this inspection. Information gathered during the inspection determined areas of noncompliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor. If you have any questions regarding this inspection, please contact the licensing inspector at (804) 929-3771.
Based on record review, the center did not ensure each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Evidence: 1. The record of staff #4 (employment date: 3/1/2022) did not have documentation showing that they have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. 2. The record of staff #5 (employment date: 9/7/2022) did not have documentation showing that they have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
Plan of Correction:
We will get the staff health reports as soon as possible.

Description:
Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children. Evidence: During the inspection of five staff records and an interview with the center representative, it was discovered there were no staff that received training in daily health screenings within the past three years. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. Daily health screenings should be completed every three years.
Plan of Correction:
We will complete the training as soon as possible.
Standard #:

Evidence: The licensing inspector did not see a carbon monoxide detector installed in the center. The center?s administrator confirmed that a carbon monoxide detector is not currently installed in the building.
Plan of Correction:
We will install a carbon monoxide detector by May 2nd.
