Comments:
An unannounced Code Compliance inspection was conducted with the Director on 10/3/22 from 9:40am to 11:00am. There were 110 children present, ranging in age from two-years to four-years-old, supervised by 18 staff. The children were observed listening to teachers read books, watching an educational television show about letters and the circus, and eating snack. Nine child records and 10 staff records were reviewed. Please complete the ?Plan of Correction? and ?Date to be Corrected? areas on the Violation Notice for each violation cited and return to be by close of business on 10/12/2022. Plans of correction should include steps to correct the noncompliance and measures to prevent the noncompliance from occurring again.
Based on review of ten staff records, the program did not obtain annual documentation that each person in a supervisory position had been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Evidence: The records for Staff #3 and Staff #4 did not include documentation of an annual health statement.
Plan of Correction:
Health screenings have been completed. Each staff member will get their Annual Health Screening each year during our staff orientation before school starts.

Description:
Based on review of ten staff records, the program did not obtain the results of a check of the central registry every five years from the date of the last check for one staff member. Evidence: The most recent central registry check on record for Staff #7 was dated 9/23/16.
Plan of Correction:
The background check has been completed. Results will be sent to the Inspector once received. The new Director will keep the files in the preschool office. She will make a chart of all staff and the dates that their background checks expire. She will check this chart every month to make sure all background checks are up-to-date.
