Technical Assistance:
Technical assistance was provided on the areas of playground safety, building maintenance, hazardous substances and program lead qualifications.
Comments:
An unannounced monitoring inspection was conducted from approximately 12:30 pm to 3:15 pm. At the time of entrance, 32 children were in direct care of 6 staff members. Ratios were in compliance. Staff and children were observed transitioning from lunch to nap, bathroom break and nap. The site was clean, organized and contained a varied supply of developmentally appropriate materials. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, required posted documents and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Based on record review, 3 out of 6 children's records did not contain documentation that, at east annually, parents confirmed that the required information in the child's record is up to date. Evidence: 1) The most recent update in the record for Child 1 was dated 02/01/2019. 2) The most recent update in the record for Child 3 was dated 01/08/2019. 3) The most recent update in the record for Child 5 was dated 04/17/2017.
Plan of Correction:
We will ask the parents to review the information and update if necessary.
