Comments:
An unannounced monitoring inspection was conducted on 5/22/24 from 10:10 a.m. to 11:11 a.m. with the Director. There were 30 children in care, ranging in age from three to five-years-old, supervised by five staff. The children were observed in circle time and outdoor play. Five child records and five staff records were reviewed. The Center has five staff with current certification in CPR and First Aid, and five staff trained in Daily Health Observation Training. One medication and the authorization form were reviewed, and the center has Four staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. The most recent Fire Inspection on file was dated 5/30/23 and the most recent Health Inspection on file was dated 1/31/24. Areas of non-compliance are identified in the Violation Notice. Based on review of five staff records, the tuberculosis (TB) screening/test form on file for a staff member did not include all required information.
Evidence: The record for Staff #3 contains a TB screening/test form that does not include the date in which the test/screening was performed.
Plan of Correction:
Staff #3 had a recent TB test file from Kaiser of which Kaiser forgot to denote the date. The staff member contacted Kaiser and obtained a dated TB test. The new dated TB results are now in the staff file and a copy is attached.
