Comments:
An unannounced complaint investigation was conducted on 6/27/24 from 10:50 a.m. to 11:50 a.m. in response to a complaint received in the licensing office regarding allegations of care, protection, and guidance of children, supervision, and forbidden actions. Based on a review of available documentation and interviews conducted, the allegation regarding supervision was determined to be valid. The allegations regarding care, protection, and guidance and forbidden actions were unable to be substantiated due to insufficient corroborating evidence. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by 5 p.m. on 7/10/24.
Based on documentation and interviews with staff, the center failed to follow required playground safety procedures including active supervision by staff to include positioning of staff in strategic locations, scanning play activities and circulating among children.
Evidence: Interviews confirmed that during afternoon playground time, when children ages two-years to six-years-old, were grouped together. Staff remained seated at a table in the shade, and did not position themselves in strategic locations and did not circulate among the children in their care.
Plan of Correction:
All staff have been retrained on playground safety and supervision. A staff playground placement chart was reviewed with all staff and copies have been placed in all classrooms as well as outside on the playground.
