Comments:
An unannounced monitoring inspection was conducted on 7/23/19 from 10:30am until 1:00pm. At the time of entrance, there were 77 children in care with 11 staff present. A sample of 8 children's records and 7 staff records were reviewed. Children were observed participating in morning activities, including story time and music and movement, eating lunch and resting quietly during nap time. Lunch service, restroom, hand washing and diapering procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and center posting were reviewed. One violation in the area of administration is identified on the violation notice and was discussed with the center director during the exit interview.
Based on record review and interview, the center did not ensure that documentation of additional immunizations are obtained once every six months for children under the age of two years.
Evidence:
1. The most recent immunization record on file at the center for child 1, age 1 year, is dated 8/16/18. Child 1 was due for an updated immunization record in February 2019.
2. The most recent immunization record on file at the center for child 2, age 1 year, is dated 8/31/18. Child 2 was due for an updated immunization record in February 2019.
3. The center director confirmed that updated immunization records for child 1 and child 2 were not obtained within the required time frame.
Plan of Correction:
The center responded with the following: The director contacted the parent of child 1 and child 2 during the inspection and requested an updated immunization record for each child.
