Technical Assistance:
Discussed Standard 8VAC20-780-60.6 - Appropriate legal paperwork shall be on file when the custodial parent requests the center not to release the child to the other parent.

Discussed Standard 8VAC20-780-440-E - There shall be at least 12 inches of space between occupied cots and rest mats.

Written information provided regarding playground fall zones.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in-person tour of the program and outdoor play area.

A renewal inspection was initiated on 11/08/2021 and completed on 11/12/2021. There were 43 children present, ranging in ages from 2 years - 5 years, with 7 staff supervising during the in-person inspection on 11/12/2021. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 children's records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 as shown in Figures 2 (Compressed Loose Fill Synthetic Materials Depth Chart) and 3 (Use Zones for Equipment) on pages 6-7 of the National Program for Playground Safety's "Selecting Playground Surface Materials: Selecting the Best Surface Material for Your Playground," February 2004, and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing.

Evidence:
1. The fall zone at the slide chutes ( two) for the outdoor play structure do not meet the requirement of four feet plus the height of the slide.
a. Both slides measure 4 feet 7 inches in height therefore, a fall zone of 8 feet 7 inches is required at each slide chute.
2. The fall zone at each slide chute measures 6 feet 6 inches which is 2 feet 1 inch less than what is required.

Plan of Correction: An AV ( allowable variance) is being requested as the slide fall zone does meet current CPSC guidelines.
