Comments:
An unannounced monitoring inspection was conducted between the hours of 9:00am and 12:00pm. Upon arrival there were 60 preschool children in care with 8 staff present. A sample of 5 children and 2 staff records were reviewed in addition to 1 medication. Children were observed playing with puzzles, coloring, other manipulatives and working with staff one-on-one.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review, the center did not ensure all required information was kept for each enrolled child.
Evidence:
Child #1 (enrolled 08/14/19) and Child #5 (enrolled 09/03/19) did not have completed emergency contacts.

Plan of Correction: The following corrective action was provided by the assistant director during the exit interview:
The parents will be contacted today and a complete emergency contact information will be recorded in each record.

Standard #: 22VAC40-185-70-A
Description: Based on a record review, the center did not have all required information for each staff person.
Evidence:
Staff #1 did not have 1 of the 2 required reference checks prior to employment. Staff #1 was with children at the time of inspection.

Plan of Correction: The following corrective action was provided by the assistant director during the exit interview:
Phone Calls have been made to the second reference, however they have not received a call back yet. The staff will be asked for a second reference to call.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
There was peeling paint along the main hallway, in reach of children.

Plan of Correction: The following corrective action was provided by the assistant director during the exit interview:
Administration is aware of the peeling paint and plan to have the walls repainted.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
The center's playground had 2 merry-go-rounds with moving parts, that were not over resilient surfacing.

Plan of Correction: The following corrective action was provided by the assistant director during the exit interview:
Administration is currently assessing the playground and equipment. Mulch will be added in the fall zones of the merry-go-rounds.
