Technical Assistance:
The following were discussed with the program director:
- identifying poison ivy
- resilient surfacing
- posting of inspections
- FA/CPR training documentation
- injury reports
Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:10 AM and departed at 12:15 PM. Forty-one children were in care. Morning activities, lunch preparation and outdoor play were observed. A sample size of five staff records and five children's records were reviewed. Medication administration and injury records were reviewed.
An exit meeting was conducted with the program director prior to closure of the inspection.
Based upon review of five children's records, the facility has not ensured that there is documentation that two or more references as to character and reputation as well as competency were checked before employment.
Evidence:
1. The record provided for staff 2 included documentation of only one reference obtained prior to employ.
2. Staff 1 verified that there was only one reference documented.
Plan of Correction:
The facility responded with the following:
Staff 2 is related to staff 1 so a second reference was not obtained. In the future at least two references will be obtained for all employees.

Description:
Based upon review of five staff records, the facility has not ensured that a completed and signed sworn disclosure or affirmation statement is obtained prior to the first day of employment at the facility.
Evidence:
1. The record provided for staff 2 indicates a date of employ of 6/19/19. There is no sworn disclosure statement or affirmation form in the file.
2. Staff 1 verified that there was no sworn disclosure and affirmation statement for staff 2.
Plan of Correction:
The facility responded with the following:
A sworn disclosure and affirmation statement will be obtained for all applicants prior to hire.
