Comments:
An unannounced monitoring inspection was completed today, September 6, 2019, beginning at 11:50 AM and ending at 2:30 PM. There were twenty three children in care today with four staff providing care and supervision and another two staff members serving other needs of the center. The children were observed playing on the playground, napping, preparing for lunch, and participating in general play. The inspector looked at the exterior and interior of the center, the playground, all classrooms and bathrooms used, and general purpose rooms. There were no medications at the center today. The inspector also reviewed drill documentation, emergency supplies, hand washing and sanitation practices. A separate file review took place on August 26, 2019, to review staff records. The inspector also reviewed a sample of five children's files. This included newly enrolled children. There were discussions about playground resilient surfacing, the physical building, and access to the basement. The violation and corrective action was discussed with center staff today. Description: Based on observation and discussion with staff, the center failed to ensure, that after the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for: Water supply and sewage disposal system.

Evidence:

1. The Report of Environmental Sanitation Inspection, posted at the facility was dated 04/16/2018. When asked if staff had a newer form, they confirmed that they did not have a new form.

Plan of Correction: Staff will continue to make attempts to contact the Health Department. Effort has been made by the facility to schedule a new inspection, per discussion.
