Comments:
An unannounced focused monitoring inspection was conducted with the center director and center staff on September 16, 2019 to determine compliance with the provisional license.The children were having rest time, eating snack, arriving from school, and playing outdoors. Snack served today: apple juice and peanut butter crackers. One staff file and seven children's files were reviewed - please see Supplemental Summary for details. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of one staff file, the facility failed to have documentation of a tuberculosis test or screening within 21 days of employment. Evidence: Staff 1 (date of hire 4/12/2019) did not have documentation of a tuberculosis test or screening on file.

Plan of Correction: Staff 1 will get a tuberculosis test or screening.

Standard #: 22VAC40-185-350-C
Description: Based on observation and staff interview, the facility failed to maintain the staff to children ratio applicable to the youngest child in the group. The staff to child ratio for 2 year old children is 1 staff for every 8 children. Evidence: At 3:00 p.m., the children wake up from rest. Staff 2 was alone with 14 children ages two years to four years. Staff 3 arrived from a bus run at approximately 3:45 p.m. and took three children to her classroom. Staff 2 was then alone with 11 children ages 2 years to 4 years.

Plan of Correction: The center was short staffed again today due to illness.

Description: Based on observation, the facility failed to maintain the 1 staff for every 18 children ages 5 years through 8 years. On 9/16/2019, Staff 4 was alone with 20 school age children, second grade through fifth grade. The youngest children in the group were seven years old.

Plan of Correction: The center was short staffed again today due to illness.

Standard #: 22VAC40-185-380-A-2
Description: Based on observation and staff interview, the facility failed to have a rest period that did not exceed two hours. Evidence: the two year old to four year olds were due to get up from rest at approximately 3:00 p.m., but did not get off their cots until approximately 3:45 p.m. due to staff shortage.

Plan of Correction: Rest time will not exceed two hours.
