An unannounced inspection was conducted on June 20, 2023 from approximately 1:30 pm to 2:10 pm in response to a self-reported incident that was reported to the Fairfax licensing office. Documentation was gathered and reviewed while at the center and a phone interview was conducted on June 29, 2023.
The preponderance of evidence gathered during the investigation doesn't support a violation of standards in regards to the self-reported incident.
