Technical Assistance:
1. Reviewed safe storage for hazardous items. 2. Recommended cot spacing be at least 12" between each occupied cot. 3. Discussed background check requirements. A copy of the current sworn disclosure statement was provided to management.
Comments:
1:05-3:25pm Religious exempt program inspection completed. Observed 54 children + 4 direct-care staff in the preschool program. Ratios and supervision were in compliance. The facility was found to be very clean and well supplied with equipment for the children. Children were napping during the inspection. Policies, procedures, and practices reviewed. Met with the Principal and Assistant Principal. Staff training requirements met. Background check non-compliance found. Corrective actions taken will be sent to the inspector for verification.
Based on records reviewed, it was determined that 7 of 7 staff records did not include a completed sworn disclosure statement. Evidence: The sworn statement include on the employment application does not include the required questions outlined on the model form. Staff records checked today that are not in compliance include: Staff #1, #2, #3, #4, #5, #6, and #7.
Plan of Correction:
Staff will completed a new sworn disclosure statement.

Description:
Based on records reviewed, it was determined that 2 of 7 staff records did not include a current central registry check (CPS), completed within the last 5 years and within the first 30-days of employment.
Evidence:
1. Staff #4 - The previous CPS check was dated as completed 9/1/05.
2. Staff #6 - Began working in August 2019, and the CPS check has not been submitted.
Plan of Correction:
CPS checks will be completed promptly.
Standard #:
22VAC40-191-50-A-3-a
Description:
Based on records reviewed, it was determined that 1 of 7 staff records did not include a completed criminal record check prior to the first date of employment. Evidence: Staff #7 - Employment began in August 2019, and the results of the fingerprinting are not on-file.
Plan of Correction:
The school indicates that the fingerprints have been completed, and they are awaiting results.
