Comments:
An unannounced Monitoring Inspection was conducted today from 9:50am to 12:00pm. There were 52 (Infants to Preschoolers) directly supervised by 9 staff. The physical plant, staff records, 6 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not ensure that physical examinations were obtained before the child's first day of attendance or within 30 days after attending. Evidence: Reviewed 6 children's records and found that Child 5 did not have documentation of a physical examination.

Based on review, the facility did not ensure that children's records were maintained with the required information. Evidence: Reviewed 6 children's records and found that 5 did not have documentation of work information and work phone number for one of the two parents listed in each record.

Description:
Based on review, the facility did not have documentation of an annual approval from the health department. Evidence: Reviewed the facility's inspection reports and found that the facility's approval expired on 12/31/19.
