Comments:
An unannounced monitoring inspection was conducted on 10/2/19 in response to a self-reported incident regarding a child being injured by a hot liquid spilling on them. Based on record review, staff interviews, and a review of the center?s policies, violations are noted on the Violation Notice. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on record review and staff interview, the center failed to ensure that the operational responsibilities of the licensee shall include, but not be limited to, ensuring that the center's activities and services are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards.
Evidence:
1. The Food, Nutrition, and Mealtime policy for employees provided by the center states that ?Staff Members should only consume water in classrooms. Coffee cups, hot liquids, or retail beverages should not be consumed in the presence of children.?
2. On 9/18/19 at approximately 9:45am, Staff #1 walked into the Two?s classroom holding a coffee mug with a lid on it, containing a hot liquid.

Plan of Correction: - Each staff member will review and will be retrained on personal items in the classroom and sign a statement of understanding that will be placed in employees personnel file for review.
(a) - Center management shall perform random unannounced supervision checks in classrooms at least twice a day to ensure compliance.
(b) During a transition of teachers. The incoming teacher will ensure all items are locked up prior to beginning shift in classroom.
- Staffs involved were coached regarding the incident. Staffs involved have thus been terminated due to previous write ups breaking policy.

Standard #: 22VAC40-185-340-A
Description: Based on staff interviews conducted, staff who were supervising children did not always ensure their care and protection.
Evidence:
1. On 9/18/19 Staff #1 walked into the Two?s classroom holding a coffee mug with a lid on it, containing a hot liquid, and placed the coffee mug on the counter by the sink. Staff #2 stated that ?Staff #1 placed their cup down on the counter and Child #1 tried to drink it and it spilled down Child #1?s chin and burned Child #1 right below the neck, on the upper chest.?
2. Staff #2 stated that after the hot liquid spilled on Child #1, the child ?was crying and screaming.?
3. Staff #2 stated that Child #1?s upper chest area looked red after the incident and stated that ?the child had 3 lines, the size of a paperclip that were visible the next day.?

Plan of Correction: - Management and Staff will go through Child Abuse and Neglect training during November?s Staff meeting. During this meeting we will discuss protection and care practices that support both the physical and emotional safety of children.
- Ms. Casey will review our Teacher Standards of Excellence with everyone and have a focus on Child Advocacy and Relationships with Children.
Teachers will be asked to sign commitments stating that as KinderCare employees they understand the companies promise to be child advocacy by everyone employed
