Technical Assistance:
Technical assistance was provided for the following standards: 22 VAC 40-185-130 (children's immunizations) and 22 VAC 40-185-330 (play areas).
Comments:
An unannounced monitoring inspection was conducted on 9/9/19 from 10:25am until 12:40pm. At the time of entrance, there were 53 children in care with 11 staff present. A sample of 6 children's records and 6 staff records were reviewed. Children were observed participating in morning activities, playing outside on the playground, eating lunch and resting quietly during nap time. Infant diapering, lunch service, restroom, hand washing and table sanitizing procedures were also observed. Medication, transportation, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. One violation in the area of administration is identified on the violation notice and was discussed with the center director during the exit interview.
Based on record review and interview, the center did not ensure that children's records contain all the required information.
Evidence: 1. The record for child 1 (date of enrollment: 6/4/18) does not contain documentation of proof of the child's identity and age. 2. The center director confirmed that proof of child 1's identity and age had not been documented.
Plan of Correction:
Center Director has contacted the parent to bring this documentation in by COB tomorrow (9/10/19) Moving forward school administration will ensure that this documentation is noted during the pre- enrollment paperwork appointment. DM will review files quarterly for compliance.
