Comments:
An unannounced monitoring inspection was conducted on 3/9/20 from 11:05am until 1:00pm. At the time of entrance, there were 63 children in care with 9 staff present. A sample of 6 children's records and 6 staff records were reviewed. Children were observed eating lunch, engaging in story time and resting quietly during nap. Restroom, hand washing, infant feeding and table sanitizing procedures were also observed. Medication, first aid and emergency supplies, transportation, documentation of emergency practice drills, children's injury reports and center postings were reviewed. One violation in the area of administration is identified on the violation notice and was discussed with the center director during the exit interview.

Description: Based on record review and interview, the center did not ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of follow-up tuberculosis screening.

Evidence:
1. The most recent tuberculosis screening for staff 1 is dated 1/31/18. An updated tuberculosis screening for staff 1 was due in January 2020.
2. The center director confirmed that the most recent tuberculosis screening for staff 1 is expired.

Plan of Correction: The center responded with the following: Staff will have a new TB test on file by 3/13/20, or be taken off the schedule. Management will develop a tickler file to ensure that all staff file expirations are managed 30 days prior to expiration to ensure compliance.
