A Subsidy Health and Safety inspection was conducted on 3/14/2024 to review supplemental health and safety requirements. There were 88 children present with 18 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
A violation was found during the supplemental subsidy health and safety inspection requirements and is identified on the violation notice.
Based on a review of eight staff records and interview, the vendor did not ensure that one staff annually completed the department's health and safety update course as required.
Evidence: 1, The record of staff #6 (date of employment: 5/30/2022) contained an annual health and safety update certificate dated 6/16/2022. The record did not contain a 2023 certificate. 2. The vendor acknowledged that staff #6 had not completed the 2023 update.
Plan of Correction:
Require all staff to complete training yearly
