Comments:
A monitoring inspection was conducted on March 25, 2024 with the center director and staff. There were 40 children present, ranging in age from 2 years to 12 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Repeat violation. Based on review of four staff records, the facility failed to ensure staff members obtained and submitted documentation of a tuberculosis (TB) test or screening at least every two years from the date of the initial screening. Evidence: Staff 3 did not have a current TB test or screening on file.

Staff 3 will get an updated TB test or screening.
