Comments:
A renewal inspection was initiated on 11/28/2023 and concluded on 11/28/2023. There were 32 children present, ranging in ages from 21 months to 6 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 4 Board Officers/Agents and 3 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-240-A
Description: Based on record review, the center failed to ensure that staff completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
1. The record of Staff 2 (DOH 8/22/2023) did not contain documentation of the Virginia Department of Education-sponsored orientation.
2. Administration acknowledged that they did not have documentation in the file.

Plan of Correction: Administration will make sure that staff have completed the training and it is in their file prior to the expiration of the 90 days after date of hire.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. The 3 to 6 year old classroom had Gillette Shaving Cream on the mantle. The label on the Gillette stated "keep out of reach of children' and "caution".
2. Administration confirmed that all the classrooms use shaving cream as an art supply and was unaware that it had to be kept locked when not in use.

Plan of Correction: Staff will begin to lock up the shaving cream when it is not in use as a manipulative or a work station.
