Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on October 21, 2019 from 10:30 a.m. through 12:30 p.m. There were sixteen children in the care of three staff in one classroom. Five children and five staff records were reviewed and updated. Areas of non-compliance may be found on the violation notice.
Areas observed were: swimming, free play, transition, lunch and nap Interactions between the children and staff were observed as positive and the children seemed comfortable with their daily routine and schedule.

Based on observation, the center failed to maintain the required six inches of resilient surfacing under the yellow slides on the outside playground.
Plan of Correction:
Pea gravel was immediately tilled and raked to ensure the six inches of depth under all fall zones.

Description:
Based on record review, the center failed to have current parent authorization for one emergency medication.
Evidence: 1. The medication and medication paperwork was reviewed for one child. 2. The parent authorization was dated 06/19/2019 3. The director confirmed the parent authorization had expired.
Plan of Correction:
An updated authorization form will be given to the parent today.
