Comments:
An unannounced monitoring inspection was conducted on 11/25/19 from 11:45am until 2:25pm. At the time of entrance, there were 46 children in care with 8 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed eating lunch and resting quietly during nap time. Infant diapering, restroom and hand washing procedures were also observed. The center does not currently have any children in care that are prescribed medication. Transportation, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the areas of administration and physical plant are identified on the violation notice and were discussed with the center director during the exit interview.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center did not ensure that staff records contain all the required information.

Evidence:
1. The records for staff 1 (hire date: 11/13/19) and staff 2 ( hire date: 11/13/19) do not contain written information to demonstrate that the individual possesses the orientation training required by the job position.
2. The center director confirmed that the records for staff 1 and staff 2 lack written documentation of orientation training.

Plan of Correction: Documentation of orientation training for staff 1 and staff 2 will be completed by the end of the day and placed in their staff files.

Standard #: 22VAC40-185-330-B
Description: Based on playground observation and measurement, the center did not ensure that when playground equipment is provided, resilient surfacing shall comply with minimum safety standards.

Evidence:
The mulch surrounding the climbing equipment and slides on the playground measures approximately 1 to 2 inches in all areas, where 6 inches are required.

Plan of Correction: The center director will put in a work order today for additional mulch.
