Comments:
An inspection was conducted today due to a self-reported concern. Staff were interviewed and classrooms were observed.
The issue involved 410: Forbidden actions. There was not a preponderance of evidence to support the concern.
However, during the investigation at the site on 8/21/2019, a violation was observed involving 350: Staff-to-children Ratio Requirements.
A Violation Notice will be submitted to the director for a Plan of Action.

Based on observation during today's inspection, one classroom was out of ratio.
Evidence:
Between 10 - 11 am today, a staff was observed alone with 26 children in the school-aged classroom. While children were occupied and involved in activities, this one staff member was also responsible for administration of the center at that time.
