11/29/22 Arrived 9:55am Departed 10:35am An unannounced monitoring inspection was conducted today. There were 15 children present with two staff. The children were working in centers and making a craft. Two child records and two staff records were reviewed. The playground was reviewed on 12/6/22.

Based on a tour of the playground, the facility failed to ensure that all equipment is in a safe condition. Evidence: On the new metal climber, there is a gap of 1/8-1/4 inch between the deck and the slides. This presents an entanglement hazard.
Plan of Correction:
The program manager will ensure that the repairs are made to the slides and deck.
