Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A (monitoring/renewal) inspection was initiated on 7/17/20 and concluded on 7/22/20. The director was contacted by telephone to initiate the inspection. There were 28 children present and 6 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Based on records review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. Child # 1's most recent immunization records were dated 1/25/19. Additional immunization records were not obtained after that date. 2. Child # 2's date of enrollment was 12/12/16. The most recent immunization records were dated 11/7/16. Additional immunization records were not obtained by the center after that date.
Plan of Correction:
The parents will be asked to provide all updated immunization records for both children.

Description:
Based on records review, the center did not obtain documentation of a physical exam for children in care within one month of enrollment. Evidence: Child # 1's first date of enrollment was 2/3/20. Documentation of a physical exam was not obtained for Child # 1 within 30 days of enrollment.
Plan of Correction:
We will request a physical exam report from the parents.
