Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A monitoring inspection was initiated on 08/02/2021 and concluded on 08/04/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 59 children present, ranging in ages from 9 months to 5 years, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, children's records did not contain all required information. Evidence: 1) The record for Child #4 did not contain work phone number and place of employment of one custodial parent.

The center shall request the parents to provide the information.

Description:
Based on record review, the center did not request annually parent confirmation that the required information in the child's record is up to date. Evidence: 1) The most recent information update documented in the record for Child #4 was dated 08/26/2019.

Description:
Based on record review and interview, it was determined that the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Central Registry search request within 30 days of employment and that the results of out-of-state Sex Offender Registry search request and out-of-state Criminal History Name Check were obtained prior to the first day of employment. Evidence: 1) The record for Staff #3 (date of hire 05/24/2021) did not contain documentation that out-of-state Central Registry search, out-of-state Criminal History Name Check and Sex Offender Registry search had been requested. 2) Staff #4 stated that required out-of-state background checks had not been requested for Staff #3.
Plan of Correction:
Will request the required out-of-state background checks for Staff #3.
