Comments:
An unannounced renewal inspection was conducted from 9:15 am-12:30. There were a total of 43 children with 9 staff members present. Children were observed during snack time, engaged in group activities and outdoor play time. A selection of staff and children records, and other documents were reviewed. Based on a review of children records, the center has not obtained documentation of additional immunization once every 6 months for children under the age of 2 years.
Evidence: Child # 2, date of birth 10/14/18 does not have an updated immunization every 6 months. Last update is 12/2018.
Plan of Correction:
I will ask the parents to provide the records.

Description:
Based on a review of children records, each child does not have a physical examination before the child's attendance or within one month after attendance.
Evidence: Child #1, date of enrollment 2/4/19 and child #2, date of enrollment 1/7/19 do not have a completed physical examination on file.

I will ask the parents to provide the physical examination for the children.

Description:
Based on a review of records, the center has not obtained a current fire inspection. The last fire inspection is dated 1/18/18.
Plan of Correction:
The Director called the Fire Marshal, an inspection has been scheduled for tomorrow 8/15/19.

22VAC40-185-280-B
Description:
Based on observation, hazardous substances are not found in a locked place.
Evidence: The storage door in the toddler classroom was found unlocked, making it accessible to the children. Hazardous substances are stored inside the closet.
Plan of Correction:
We will remind the teachers to keep the door locked at all times.
