Technical Assistance:
During inspection staff placement in the gym as indicated on the injury prevention plan was discussed. Also medication and planning documentation was also reviewed and discussed.
Comments:
An unannounced renewal inspection was conducted today from approximately 4:40pm-6:00pm. There were 29 children (school age) directly supervised by 4 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books and playing in the gym. There was an adequate supply of materials for the children. The required number of staff with current certification in MAT, CPR and First Aid, as well as DHO training were present. Staff-to-child ratios were in compliance. Based on record review, An employee of a licensed or registered child welfare was employed without a completed sworn statement or affirmation. Evidence Staff A did not have a sworn statement of affirmation on file. Staff A is board officer of the organization.
