1. Number 3 on the Campagna Center's "Procedures for Face to Name Sheets" under the "Steps for filling out face to name sheets during transition" states "As each child leaves the classroom look at the child's face, then find their name on the face to name sheet and place a check mark next to the child's name. Do this for every child as they leave the classroom"
2. Staff A confirmed by signature that these procedures were covered during orientation on 11/31/2016.
3. Staff B confirmed by signature that these procedures were covered during orientation on 01/15/2019.
4. Staff A and B confirm that they did not follow the above procedures.

Description:
Based on staff interviews, children under 10 years of age were not always maintained within actual sight and sound supervision of staff.
Evidence:
1. Staff interviews confirmed that on 06/11/2019, Child A (5 years old) was left unsupervised in the bathroom located in the classroom for an undetermined amount of time when the group transitioned out of the classroom.
2. Staff A stated the class left the classroom at approximately 11:15am to attend an unscheduled graduation rehearsal located in the gymnasium.
3. Staff B stated the class left the classroom at approximately 10:20am to attend an unscheduled graduation rehearsal located in the gymnasium.
4. Staff C stated that Child A was found unattended in the bathroom at approximately 11:20am.
5. Child A was taken to the gymnasium to be reunited with his class by Staff C at approximately 11:20am.
6. During an interview conducted 06/11/2019, Staff A and B stated they were unaware that Child A was missing from the their group.
