Comments:
An unannounced monitoring inspection was conducted today from approximately 2:05pm-3:10pm. There were 47 children (preschool) directly supervised by 8 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books and eating snack There was an adequate supply of materials for the children. There were no violations observed during this inspection.

Based on record review, An employee of a licensed or registered child welfare was employed without a completed sworn statement or affirmation. Evidence Staff A did not have a sworn statement of affirmation on file. Staff A is board officer of the organization.
