Comments:
An unannounced monitoring inspection was conducted today between the hours of 11am- 2:15pm. 30 children were observed directly supervised by 6 staff. The physical plant, 3 staff records, 3 children?s records, medications, evacuation drills, and emergency supplies were inspected. Children were observed washing hands and preparing for lunch. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-60-A
Description: Based on record review, 1 of 3 children's records reviewed did not contain all required information.

Evidence:
1. The record for Child #1 did not contain the following:
-documentation of the child's first date of attendance
-the name, address, and phone number of the mother's place of employment
-Name, address, and phone number of one of two designated people to call in an emergency if a parent cannot be reached

Plan of Correction: We will contact the enrollment specialist today.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas inside the center were not maintained in a safe condition.

Evidence:
Peeling paint was observed on the back wall of Classroom 134, accessible to children.

Plan of Correction: We will contact the supervisor today to put in a work order.

Standard #: 8VAC20-780-510-G
Description: Based on observation, the center did not ensure medications were labeled.

Evidence:
1. Medications for Child #2 and Child #3 were not labeled with the child's name.

Plan of Correction: Corrected during inspection.
