Comments:
An unannounced monitoring inspection was conducted today between the hours of 11am- 2:15pm. 30 children were observed directly supervised by 6 staff. The physical plant, 3 staff records, 3 children?s records, medications, evacuation drills, and emergency supplies were inspected. Children were observed washing hands and preparing for lunch. Areas of non-compliance are identified in the Violation Notice. Based on record review, 1 of 3 children's records reviewed did not contain all required information.
Evidence: 1. The record for Child #1 did not contain the following: -documentation of the child's first date of attendance -the name, address, and phone number of the mother's place of employment -Name, address, and phone number of one of two designated people to call in an emergency if a parent cannot be reached
