Comments:
An unannounced inspection was conducted on December 5, 2023, from 9:30 AM until approximately 10:50 AM. There were 7 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Staff records were reviewed remotely.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review and interview, the center failed to provide evidence of current liability insurance. Evidence: 1. The liability insurance that was provided had an expiration date of 9/17/2022. 2. Staff # stated that the liability insurance had been renewed but she had not been sent a copy of the new policy.

Will check the status of current policy with administrator.

Description:
Based on record review and interview, the center did not ensure that an annual fire inspection report from the appropriate fire official having jurisdiction was updated. Evidence: 1. The fire inspection posted was dated for 5/31/2022. 2. Staff # stated that a fire inspection had been completed but did not have the inspection report.

Fire inspection was conducted on May 25, 2023, didn't receive paperwork.

Description:
Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence: 1. The wood paneling on the wall under the window and on the wall near the door in the toddler classroom, had 2 nails protruding which could snag skin or clothing.
Plan of Correction:
Will remove nails and secure panels against the walls.
