Comments:
An unannounced monitoring inspection was conducted on 11/29/2022. There were 5 children, ages 16 months ? 3 years, totaling 12 points and 4 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records, 2 caregiver records and 1 household member?s records were reviewed. The children were observed during center time, handwashing and snack time. The inspector arrived for the inspection at 10:20am and departed at 12:07pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on the review of one pet record, the provider failed to maintain documentation of the current rabies vaccination.
Evidence: The most recent rabies vaccination certificate expired on 5/28/2022 and on the day of the inspection, no up to date rabies vaccination was documented.
Plan of Correction:
Provider will update rabies vaccination and obtain documentation from veterinarian.
