Comments:
An unannounced monitoring inspection was conducted on 5/11/2023. There were 9 children, ages 20 months ? 6 years, totaling 15 points, 2 caregivers and 2 household members present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records, 1 caregiver record and 1 pet record were reviewed. The children were observed during nap time and snack time. The inspector arrived for the inspection at 2:30pm and departed at 4:20pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on the review of one caregiver record, the provider failed to obtain for each caregiver a current report of tuberculosis screening form.
Evidence: Caregiver 1 most recent tuberculosis screening was dated 7/31/2018 and on the day of the inspection no tuberculosis screenings were documented for 2020 or 2022.
Plan of Correction:
Provider will schedule a tuberculosis screening for caregiver.

Evidence: Licensing staff observed children eating at snack time and children?s hands were not washed before eating.
