Comments:
An unannounced renewal inspection was conducted on 12/15/2023. There were 2 children, ages 22 months ? 2 years, totaling 5 points, 2 caregivers and 2 household members present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 2 children?s records, 2 caregiver records, 2 household member records and 3 pet records were reviewed. The children were observed during center time. The inspector arrived for the inspection at 9:45am and departed at 11:20am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review, the provider failed to ensure 5 year repeat record checks were completed.
Evidence: 1. Provider 1 most recent criminal background check was dated 3/7/2018 and on the day of the inspection the 5 year repeat record check was not completed. 2. Household Member 2 most recent criminal background check was dated 3/9/2018 and on the day of the inspection the 5 year repeat record check was not completed.
Plan of Correction:
Provider has scheduled for criminal back results to be obtained.
