Comments:
A monitoring inspection was initiated on 513/2022 and concluded on 5/17/2022. There were a total of 69 children with 15 staff members present. Children were observed in free play activities, outdoor play, circle time for reading and color of a butterfly.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-270-A
Description: Evidence:
1. Based on observation in the classroom of Preschoolers, the toilet seat was chipped and with no toilet lid. Wet and soiled tissue were in the corners of the wall and red stains on the floor.
2. Based on observation, a changing pad had visible tears.

Evidence:
1. Director will discuss with staff to follow cleaning checklist. Closing staff will help assist with bathroom cleaning. A work order has been sent to maintenance for repairs.

2.Changing pad was observed with tears in the waterproof cover.

Plan of Correction: Worker order was submitted to center's maintenance multiple times.
Director will ensure staff are following cleaning checklist for bathrooms. Closing staff will assist when needed to keep the bathrooms clean.

Standard #: 22VAC40-185-510-N
Description: Evidence:
1. Child #1 over-the-counter mediation expired in April 2022.
2. Child #6 over-the-counter medication expired in September 2021.

Plan of Correction: Director discussed with staff each child have current authorization forms, labeling of medication and returning medication back to parent once expired.
