Technical Assistance:
Adequate space in the toddler room was discussed with the program director.
Comments:
A renewal inspection was conducted on (07/09/2024. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed children and staff records along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a record review the center did not ensure the current immunization record form was signed by a physician, his designee, or an official of a local health department.
Evidence: Child #2 did not have an immunization record that was signed by the physician and did not indicate if the child had been adequately immunized.
Plan of Correction:
The child has had their immunizations, however the parent did not have the physician sign the form the practice gave to them. The director will request a signed immunization record from the child's parent today at pick-up.

Description:
Based on a record review the center did not ensure all required information was kept for each enrolled child. Evidence: Child #2 did not have documentation of 2 complete emergency contacts.

The director will obtain the complete address and contact number for child#2's emergency contact.

Description:
Based on observation and measurement the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence: The center had 2 composite play structures on their playground, requiring at least 9 inches deep of resilient surfacing at least 6 feet around the structure. The preschool playground had approximately 6 inches deep and the school-age playground had approximately 5 inches deep of wood chips as the resilient surfacing in the fall zones.
Plan of Correction:
The center has been maintaining the surfacing, however recent rain and weather has impacted the surfacing. The mulch has been escalated and is due to be installed on Thursday, weather permitting.

Description:
Based on observation the center did not ensure all vehicles used for transportation of children met all the requirements. Evidence: One of the center's vans had an expired license plate date 04/2024.
Plan of Correction:
The director contacted maintenance during the inspection. She believes the stickers had been stolen. Maintenance will ensure new stickers are obtained and the vehicles license plates are current.
