Comments:
A monitoring inspection was conducted on May 26, 2023 with the center director and staff. There were 31 children present, ranging in age from 2 years to 12 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed. The children were waking from rest, eating afternoon snack, and playing outdoors.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and staff interview, the facility failed to ensured that the diaper changing area is equipped with a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The diaper disposal trashcan in the bathroom was not equipped with a lid. Staff acknowledged that there was no lid on the trashcan where diapers were disposed.

Diapers will not be disposed of in an open trash can.

Description:
Based on review of two prescription medications, the facility failed to have written authorization from the parent and the child?s physician to administer medication. Evidence: There was no written parent or physician authorization to administer one prescription medication to one child. Staff confirmed that Child A did not have a completed medication authorization on file.
Plan of Correction:
The medication authorization will be obtained from the parent or the medication will be sent home.
