Based on record review, the center did not ensure one of five staff records contained documentation of a tuberculosis screening at the time of employment and before contact with children. The documentation should have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: The record if staff #2 (start date: 8/19/19) had documentation of TB results completed in May of 2019.
