Technical Assistance:
1. Potentially hazardous substances, such as cleaning supplies, are to be stored in a manner that prevents access by the children. If the product label states "keep out of reach of children" then the product should be stored in a place that children cannot reach. 2. The inspector will send information about obtaining out-of-state background checks.
Comments:
Conducted the annual religious exempt renewal inspection at 10:00am. Observed 13 children + 3 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities to include: reading books, washing hands, eating snack and reading books. Staff were actively engaged with the children and activities in all groups. The facility was found to be clean and well maintained inside and outside. Classrooms were well supplied with toys, supplies and equipment for the children. Based on records reviewed, 1 of 2 staff records did not include documentation of an in-state background check every 5 years. Evidence: The most recent VA Central Registry Search was dated as completed 11/20/18.

I've given all staff forms to update. I'd like to have them completed by mid-May.

Description:
Based on records reviewed, 1 of 2 staff records did not include documentation of out-of-state background checks.
Evidence:
1. The Sworn Disclosure Statement signed by Staff #2 on 9/18/18 indicated she had lived out of the State of VA in the previous 5 years and there is no documentation of the out-of-state Central Registry Search being completed to-date. 2. Additionally, there is no documentation of the out-of-state Criminal Record Check being completed to-date.
Plan of Correction:
She is getting the necessary documentation from D of C. I had no idea this was necessary. The employee is planning to go to get + submit completed form.
