Comments:
The licensing inspector conducted an unannounced inspection in response to a complaint that was received by the department on November 13, 2019 regarding supervision and procedures for emergencies. The inspection was initiated on November 14, 2019 from approximately 5:30 pm until 6:25 pm. Staff interviews were conducted, classroom observations made and documentation reviewed. The preponderance of the evidence gathered during the investigation supports portions of the allegation and therefore the complaint regarding procedures for emergencies is determined valid. Based on review of injury reports, the facility did not maintain a written record of children's serious and minor injuries in which the record included all required elements.
Evidence:
1. The injury report for child #1, dated 11/12/2019, did not contain documentation of the date and time when the the parent was notified. 2. The injury report for child #1, dated 11/12/2019 had the signature of one staff but did not have the signature of the parent or a second staff member as required.
Plan of Correction:
Per the director: The staff was upset about the situation and did not complete the incident report form. Administration will follow up with staff in the future to ensure all components are completed.
