An unannounced monitoring inspection was conducted to investigate a self reported incident regarding care, protection, and guidance, hand washing and toileting. The inspection was conducted with the director on August 16, 2023 beginning at 1:50pm and ending at 2:50pm. Three staff interviews were conducted and facility documentation was reviewed. The allegations were unable to be substantiated due insufficient corroborating evidence.
