An unannounced inspection was conducted on Friday, November 15, 2019 in response to a self-report received by the Department on October 18, 2019 regarding a serious injury. The inspection was initiated at 12:20pm and concluded at approximately 12:35pm. Interviews were conducted, documentation was reviewed, and observations of the outdoor playground were completed. There were no violations found during this inspection.
