The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 10, 2023 from approximately 10:30 AM to 1:30 PM. There were 59 children in attendance and a total of 18 staff supervising. A total of six children and six staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of six staff records, the vendor did not ensure three staff were evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the required timeframe. Evidence: 1. The record for Staff #1 (Date of Employment 11/21/2022) contained a TB screening dated 12/05/2022. 2. The record of Staff #3 (Date of Employment 12/12/2023) contained a TB screening dated 09/22/2023. 3. The record of Staff #4 (Date of Employment 09/05/2023) contained a TB screening dated 09/22/2023. 4. The documentation of the TB screening is required to be obtained within the last 30 calendar days of the date of employment and be signed by a physician, physician?s designee, or an official of the local health department.
Plan of Correction:
Going forward we will ensure all staff members begin employment with TB screenings obtained within the required timeframe.
