Comments:
A renewal inspection was conducted on 07/21/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed 5 children?s records and2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a record review the center did not have all required staff information. Evidence: Staff #2 did not have the written documentation of having the qualifications for her position as a program leader.
Plan of Correction:
Staff #2 has a bachelor's degree and does qualify for the program leader position. The director will have staff #2 obtain a transcript.

Description:
Based on observation the center did not ensure all requirements were met on the playground. Evidence: The slide had a ground support that was not covered with resilient surfacing.
Plan of Correction:
The center is closing for a month for summer, The ground covers will be covered with resilient surfacing prior to children returning to the building.
