Comments:
An unannounced monitoring inspection was conducted today from 10:05am-11:45am. There were 55 children (infants-5yrs.) directly supervised by 14 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. There was a supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's records it was determined that all required information was not maintained.

Evidence: During inspection: Child 1 and Child 2 were missing documentation of annual updates to files. Child 3 and Child 4 were missing documentation of acknowledgement of emergency plan

Plan of Correction: Director will contact the parent(s) to get the required information.

Standard #: 22VAC40-185-270-A
Description: Based on inspection of the facility it was determined that areas were not safely maintained.

Evidence: During inspection:
1. Radiator vents were left uncovered in the toddler classroom.
2. Paint and wallpaper was peeling in the bathroom used by the preschoolers.

Plan of Correction: Director stated that they have notified the building maintenance staff.

Standard #: 22VAC40-185-280-B
Description: Based on inspection of facility it was determined that all cleaning supplies and hazardous substances were not kept in a locked place.

Evidence: During inspection a spray bottle marked "bleach" was kept on top of the windowsill in the pre-school classroom.

Plan of Correction: Teachers moved the bottles during inspection.
