Comments:
The Licensing Inspector for Life Church of the Roanoke Valley (Life Academy) conducted an unannounced Monitor Inspection on Friday, February 7, 2020 Licensing Inspector arrived and started the inspection at 10:00 AM. There were seventy-two (72) children ages 5 months to 5 years old and fourteen (14) staff members present during today?s inspection. The Licensing Inspector reviewed 5 staff and 10 children records. Children were observed during arrival/departure procedures, resting/nap time, bathroom/hand washing/diapering procedures, snacks/meal served and infant feeding, group activities & self-initiated activities.
The Licensing Inspector conducted an exit interview on February 7, 2020. Areas of non-compliance and the Risk Assessment were reviewed with the Center Director. Inspection concluded at 4:15 PM. If you have any questions, the Licensing Inspector for this facility can be reached at (540) 309-2397.
Based on review of staff information, the Center failed to ensure each employee, volunteer or any other person who is expected to be alone with one or more children enrolled in a religious exempt child day center has completed Central Registry Background Check within 30 days of employment or commencement of service.
EVIDENCE: Licensing Inspector information for five (5) staff members. Information for Staff #1 did not include evidence of completed Central Registry (CPS) Background Check.
Plan of Correction:
The program will send documentation to the Licensing Inspector that the background checks have been requested no later than 10 days following this notification.

Description:
Based on review of center information, the Center failed to provide a written statement regarding physical facilities, health requirements for the staff and public liability insurance to the parents or guardians of the children in the center and made available to the general public.
EVIDENCE: A written description of the center's physical facilities, information regarding the center's health requirements for staff and information regarding the center's public liability insurance were not included in the parent handbook nor posted.
Plan of Correction:
Center will add required written disclosures to the parent handbook.
