Comments:
Two Licensing Inspectors arrived at 12:05 PM and conduct an unannounced Religiously Exempt Code Monitor inspection on April 22, 2024. The Licensing Inspector reviewed 3 children?s records and 9 staff record. The inspection included record reviews, interviews with staff, and observations.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.
Inspection concluded at approximately 2:45 PM.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Centers must ensure fingerprint background checks are received for each staff person prior to employment. The center did not have a fingerprint background check for Staff #9 who has been employed for approximately 2 years.
